1. M.B.B.S. Degree or Provisional Pass Certificate.
2. Statements of marks of all M.B.B.S. Examinations
3. Caste Certificate (if SC/ST/OBC).
4. Compulsory Rotating Internship Certificate.
5. State Medical Council Registration Certificate.
6. Conduct Certificate from the Head of the Institution from where the candidate completed the M.B.B.S. Course.
7. Post-graduate diploma certificate, if any.
8. For former students of this Institution, a certificate of service in rural and underserved areas in this country issued by the Director of SJNAHS in the format given above.
9. NEET (PG) 2025 - Results
10. Transfer certificate (Students from colleges other than St. John's Medical College)
11. Migration certificate and Eligibility certificate (Students from Universities other than RGUHS) 12. SJMC Social Obligation Service as stipulated under 3.1.3 on stamp paper
13. Photocopies of certificates showing distinctions, scholarships, honours, medals at MBBS and extracurricular activities at Inter collegiate or State level, if any.
14. Six passport size photographs
